Overcoming Terminal Dentition Case Study: A Full-Mouth All-on-4 Transformation

The Challenge

Our patient arrived in significant distress, struggling with chronic tooth pain and a smile he went to great lengths to hide.

His existing dental work was failing, and a combination of advanced decay and bone loss had rendered his remaining teeth non-restorable.

He wasn’t just looking for an aesthetic upgrade; he needed a functional reset to eat and live without pain.

The Solution

Following a comprehensive diagnostic phase—including a 3D CBCT scan to assess bone density and a detailed facial analysis—we diagnosed him with terminal dentition.

To provide the most stable and long-lasting result, we recommended a Full-Mouth All-on-4 protocol.

This approach allows us to utilize the available bone to anchor a fixed bridge, eliminating the need for extensive bone grafting in many areas.

Visit 1: The Foundation & Immediate Function

On the day of the procedure, all compromised teeth were gently extracted, and four high-stability implants were strategically placed in each jaw.

  • The Temporary Phase: We immediately began crafting high-quality temporary dentures. Within a few days, the patient was fitted with these restorations, allowing him to leave our clinic with a functional smile while his implants underwent osseointegration (the 4-month healing process where the bone fuses to the implants).

Visit 2: Precision Engineering & The Zirconia Final

Four months later, the patient returned for his permanent transformation.

  • The Digital Prototype: We captured high-precision digital impressions to design a functional plastic prototype. This “test-drive” allowed the patient to verify his bite (occlusion) and aesthetic preferences in real-world conditions.
  • The Final Masterpiece: Once the design was perfected, our in-house lab milled a custom All-on-4 Monolithic Zirconia superstructure. Five days later, the final bridges were seated.

The Delivery

After a final 24-hour test period and minor bite refinements, the patient was delivered his final mouthguard and x-rays.
